Cervical subarachnoid floating cavernous malformation presenting with recurrent subarachnoid haemorrhage.

چکیده
A61 year old man was admitted to our department due to recurrent episodes of subarachnoid haemorrhage (SAH). He had been previously healthy except for a 10 year history of hypertension. One year ago, SAH had occurred but four vessel cerebral angiography showed no cause of SAH. One month ago, an intradural extramedullary (IDEM) lesion was detected due to right posterior auricular pain (fig 1A). On admission, even though brain computed tomography did not detect SAH, lumbar tapping showed SAH. Because, four vessel cerebral angiography demonstrated no evidence of vascular abnormalities, we explored cervical lesion. At surgery, a deep violet floating strawberry-like lesion was found above dorsal surface of spinal cord and was attached to the cord with a vascular pedicle (fig 1B). The lesion was removed en bloc without postoperative neurologic deficit. Pathologic examination revealed a cavernous malformation and an old haemorrhage. Dilated vascular channel lined by endothelial cells without intervening brain parenchyma suggests cavernous malformation rather than arteroivenous malformation (fig 2). Recurrent SAH caused by a cervical IDEM cavernous malformation is extremely rare and only one case has been reported till now. To the best of our knowledge, the present case is only the second reported case.

The efficacy of resection of an intradural extramedullary foramen magnum cavernous malformation presenting with repeated subarachnoid hemorrhage: a case report
BACKGROUND Intradural extramedullary cavernous angiomas of the central nervous system are a rare type of cavernous angioma, but they can cause fatal subarachnoid hemorrhage. The efficacy of resection for this type of cavernous malformations remains uncertain. This is the first report to recommend surgical resection of these types of lesions regardless of the fatal condition. CASE PRESENTATION...
متن کاملIsolated angiitis of the CNS presenting as subarachnoid haemorrhage.
Isolated angiitis of the CNS (IACNS) commonly presents with recurrent ischaemic or haemorrhagic infarcts, but subarachnoid haemorrhage is rare. Three patients with IACNS and subarachnoid haemorrhage are reported. Florid granulomatous angiitis with Langhans and foreign body type giant cells was found at necropsy in a child with sudden death. In two other patients the diagnosis was made angiograp...
متن کاملSimultaneous presentation of two cerebral arterial aneurysms.
A case of subarachnoid haemorrhage due to rupture of a middle cerebral artery aneurysm followed by immediate enlargement of a carotid intra-cavernous aneurysm is described. The enlargement of the second aneurysm was due to a rise in blood pressure caused by the subarachnoid haemorrhage. Although multiple aneurysms are common, this phenomenon is very rare and the reasons for this are discussed.
متن کاملSubarachnoid haemorrhage due to upper cervical trauma.
Seventeen cases are reported in which fatal subarachnoid haemorrhage was associated with injury to the upper cervical region. Most of these cases were alcohol-intoxicated, most had sustained their injuries in an altercation, and death was usually but not invariably rapid. It is proposed that trauma to the upper cervical region can cause subarachnoid haemorrhage, by a mechanism involving trackin...
متن کاملDelayed Positivization of Cerebral Angiography in Reversible Cerebral Vasoconstriction Syndrome (RCVS) Presenting with Recurrent Subarachnoid Haemorrhage
Benign angiopathy of the central nervous system is a clinical syndrome with evidence of reversible cerebral vasoconstriction (RCVS). Haemorrhagic stroke, either subarachnoid or intracerebral is an unusual presentation of RCVS. We describe a case of RCVS presenting with a subarachnoid haemorrhage (SAH), with rebleeding and onset of hydrocephalus during the first week, and, notably, delayed evide...
